A Tasmanian coroner determined that a former doctor's grossly irresponsible prescribing of opioids directly caused the deaths of at least two drug-dependent patients between September 2016 and August 2017. The practitioner, who faced multiple red flags, had provided what the coroner described as 'effectively an unlimited supply for a drug binge' to at least one victim.
